Registered Nurse – Part Time
POSITION SUMMARY:
The RN will assist Telespond Senior Services in providing high quality care services to Home Care, Adult Day Program, and Older Adult Advocacy Center clients by supporting the organization with client centered care planning, client intakes and assessments, on-going monitoring of client care, support coordination for clients and orientation and training of care staff as needed as well as required health screenings for hiring purposes as needed.
ESSENTIAL FUNCTIONS:
1. Complete client home visits for initial intake assessments, admissions, on-going monitoring, incident investigations, and to train field staff as needed throughout Lackawanna County and neighboring counties as needed.
2. Complete all necessary paperwork required for the provision of care services such as the client service/care plan assessment, health history, and all other relevant forms.
3. Initiate and maintain all client charts and adhere to all HIPAA, Department of Health and Department of Aging regulations.
4. Quality assurance of all charts to ensure appropriate documentation and required updates are being completed well and in a timely manner.
5. Communicate with physicians and families to correctly document all client changes and care needs and chart all relevant data regarding health services rendered.
6. Provide timely response to all intake inquiries; works closely with all staff define care staff assignments, program placement, and ensure accurate care delivery.
7. Provide nursing care to clients as needed to include medication administration, vital checks, dietary supervision, catheter/colostomy care as well as health screenings for staff as required such as Tuberculosis screenings.
8. Assists in maintaining a safe and hygienic care environment for all clients.
REQUIRED QUALIFICATIONS:
1. Current and Active RN License in the state of PA.
2. 2 years’ Previous experience providing nursing care and/or care management to older adults.
3. Willingness to travel 50% . Must drive and own a PA state compliant and reliable means of transportation.
The Registered Nurse must be:
